It's really simple actually, you can see the design in the video at certain points when it bobs.
The basics is simply 4 of those square PHX-frames (with holes in the middle). Then you put them nocollide them with each other, and make them overlap.

Kind of like that (illustrated as two opposing frame squares, 0 and X), basically you make a larger and thicker frame from the 4 separate frames, make it slightly tighter than the monorail-track itself so it grabs. Then simply weld them all together. Voila, duplicate and attach them to some base structure and it should all work.
Look in the video (0:13-0:15 is a good spot) and you'll probably see more precisely what I mean (also, try different physics properties).
It can take quite a lot of tweaking to get them just right, I think I redid it like 10 times, changing the size of the hole and making the fit better. Either way, when aligning the pieces, do it manually, it's not really hard, you could also use a plate (to assist) to keep them level, but I didn't.
EDIT: Wait I just checked the video, I described the first version of the train, either way, it's the same idea, just that instead of being aligned in the corners as a bigger box, they're aligned at the top/right/bottom/left, forming a "hospital" symbol.
